Runner's Knee

Why the problem starts above and below the joint.

For many athletes, the knee is where the pain screams the loudest, but it is rarely where the problem starts. "Runner's Knee" (clinically known as Patellofemoral Pain Syndrome) is a classic example of how our bodies are a series of interconnected levers. When one part of the machine is out of alignment, the "hinge" in the middle (the knee) pays the price.

The Knee: A Victim of Its Neighbors

The knee is essentially a stable joint caught between two very mobile neighbors: the hips and the ankles. In a perfectly functional stride, these joints work in harmony to absorb the shock of each step. However, when muscular imbalances exist above or below the knee, the kneecap (patella) no longer tracks smoothly in its groove.

Instead of moving straight up and down, the patella begins to rub against the thigh bone, leading to localized inflammation, swelling, and that familiar dull ache behind the kneecap.

The Root Causes: Hips and Lower Legs

To fix the knee, we must look at the anchors that control its position:

  • The Hip Powerhouse: The glutes and hip abductors act as the "steering wheel" for your leg. If your hips are weak or misaligned, your thigh bone (femur) can rotate inward every time you land. This "collapsing" motion forces the knee into an unnatural angle, creating immense pressure on the joint.
  • The Lower Leg and Foot: Your lower legs and feet are your body's primary shock absorbers. If there is an imbalance in the calves or if your arches collapse (overpronation), the rotational force travels upward, twisting the lower leg and pulling the knee out of its functional track.

The "Time Under Tension" of the Stride

Running is the ultimate expression of the time under tension rule. If you take 1,000 steps a mile with a slight hip misalignment, you are essentially "training" your body to move in a compromised position. Your nervous system gets good at this inefficient movement, reinforcing the very imbalances that cause the pain.

Over time, this isn't just a "sore knee." It's a systemic failure of the kinetic chain.
